This registration form must be filled out for each dancer enrolling in any of our classes in order to secure their spot in class. After registering for class, you will receive a monthly tuition invoice through email, processed via Square, due by the first of each month.
To register for Adult Beginner or Kid & Kin classes, please refer to those specific information pages! These classes are run in partnership with Middleton Parks & Recreation and registration is processed through their website.
If you have more than one dancer in your household, please submit a separate form for each dancer.
