This registration form must be filled out for the following classes:

Adult Intermediate/Advanced

You must fill out a new registration form for each dancer enrolling in classes.

Adult Beginner - Tuesdays, 6:30-7:25 PM

Adult Intermediate/Advanced - Tuesdays, 7:30-8:25 PM

All payments to Madison Irish Dance are invoiced through Square.

Tuition is billed the first of each month.

Tuition and registration fees are non-refundable and non-transferable.

Should you choose to pause classes, Madison Irish Dance must receive 60 day notice via email at molly@madisonirishdance.com

Tuition that is 5 days past due is subject to a $10 late fee.

I understand that I will receive an invoice from Madison Irish Dance via Square for the annual registration fee of $45 once my registration form is received. I agree to pay this fee in order to reserve my spot in class. A $10 late fee will be applied if the registration fee is not received before my first class. Required
